Research has shown that intrinsic motivators of physical activity that satisfy an innate psychological need (e.g., skill development, satisfaction, self-worth, enjoyment) better reflect increased and maintained physical activity than extrinsic factors that are executed to achieve a separate consequence (e.g., praise, money, increased energy, improved quality of life. One factor that helps to motivate participation in fitness programs involves feeling steady on ones feet while exercising. If a person struggles with the fear of falling while exercising, they are much less likely to continue with a physical activity program. The goal of the National Robotics Initiative  project is to develop a four-legged robot that enhances mobility, so that the elderly can remain physically active and enjoy a healthier life with reduced reliance on the assistance of caregivers or expensive home renovations.  The robot has two modes: smart power-assist walker and smart mule. In the smart power-assist walker mode, the user is situated within the robot and chooses the amount of powered assistance that is needed. In the smart mule mode, the robot walks alongside the user while carrying a load, for example groceries. The robot uses a 3-D computer vision-based sensing system to detect the user’s motion and the environment. With its smart legs, the robot is able to easily overcome environmental obstacles in ways that powered wheelchairs cannot. In order to most effectively use the robot, older adults are encouraged to perform certain senior balance exercises to first develop appropriate stability.

Balance problems and falls are common among the elderly and are a leading cause of institutionalization in this group that result in over five million patient outpatient visits per year.  It is estimated that between 28% and 35% of individuals over age 65 fall each year, with a fifth of those requiring medical attention. The number of fallers increases to over 40% for those 75 and older. Low back pain  is the most frequently reported musculoskeletal condition in the elderly, with a prevalence ranging from 19.7% in people over the age of 65, to as high as 40 % for individuals over 75.  Low back pain is a leading comorbid factor directly linked to the incidence of falls in the elderly and is sharply on the rise; Low back pain prevalence has increased in the last fourteen years from 3.9% to 10.2%.  Low back pain in the elderly can involve a wide range of possible diagnoses and co-morbidities, including a high incidence of malignant or visceral causes and therefore necessitate a close review of systems in addition to the usual musculoskeletal examination.
