The differential diagnoses of knee chronic pain and osteoarthritis include: conditions involving soft tissue of knee such as bursitis, iliotibial band syndrome, ligamentous instability (medial and lateral collateral ligaments), meniscal pathology; other forms of arthritis like gout and pseudogout, rheumatoid arthritis and septic arthritis; referred pain from neuropathy or radiculopathy; and other diagnoses such as avascular necrosis, patellofemoral pain syndrome and tumor. Diagnostic criteria for osteoarthritis have been developed by the American College of Rheumatology.  Classification of knee osteoarthritis has been established by the  Kellgren and Lawrence grading scale. This scale is based on radiological imaging and consists of different grades:
Grade 1: doubtful narrowing of joint space and possible osteophyte lipping;
Grade 2: definite osteophytes and possible narrowing of joint space;
Grade 3: moderate multiple osteophytes, definite narrowing of joint space and some sclerosis and possible deformity of bone ends; 
Grade 4: large osteophytes, marked narrowing of joint space, severe sclerosis and definite deformity of bone ends.

Clinical criteria
  – Age older than 50 years
  – Bony enlargement
  – Bony tenderness
  – Crepitus
  – No palpable warmth
  – Stiffness for < 30 minutes
Laboratory criteria
  – Erythrocyte sedimentation rate  < 40 mm/hour
  – Rheumatoid factor < 1:40
  – Synovial fluid analysis:  clear, viscous, white blood cell count < 2,000/μL (2.00 × 109 per L)
Radiographic criteria
  – Presence of osteophytes

In summary:  Twenty studies which investigated the effects of multi-component exercise training (10), resistance training (6), endurance training (1) and balance training (3) were included in the present revision. Ten trials investigated the effects of exercise on the incidence of falls in elderly with physical frailty. Seven of them have found a fewer falls incidence after physical training when compared with the control group. Eleven trials investigated the effects of exercise intervention on the gait ability. Six of them showed enhancements in the gait ability. Ten trials investigated the effects of exercise intervention on the balance performance and seven of them demonstrated enhanced balance. Thirteen trials investigated the effects of exercise intervention on the muscle strength and nine of them showed increases in the muscle strength. In conclusion, the multi-component exercise intervention composed by strength, endurance and balance training seems to be the best strategy to improve rate of falls, gait ability, balance and strength performance in physically frail older adults.

Holding another person's hand or using an assistive device while walking are simple interventions that are used naturally to address poor balance confidence in daily life. However, more complex interventions have also been developed and tested to achieve more sustained improvement in balance confidence.  Results: Five of the 8 interventions that directly targeted balance confidence showed benefits. Interventions that targeted elderly persons reporting poor balance confidence and/or those at risk for falls seemed more likely to be beneficial. Vestibular dysfunction can become increasingly more frequent as people age. On another note, after acute stabilization of the patient with vertigo, use of vestibular suppressant medications should be minimized to facilitate the brain’s adaptation to new vestibular input. Lately, medical researchers have proven the effectiveness of rehearsing exercises to improve dizziness and imbalance. As an example, A randomized, controlled trial of 143 primary care patients with dizziness and vertigo showed that vestibular rehabilitation exercises improved nystagmus, postural control, movement-provoked dizziness, and subjective indexes of symptoms and distress. Another randomized, controlled trial evaluated the effectiveness of home vestibular rehabilitation in patients with chronic vertigo with a peripheral vestibular etiology. This trial showed a significant reduction of vertigo and an increase in the ability to perform activities of daily living independently. A retrospective case series assessed the efficacy of physical therapy in patients who had vestibular and balance disorders with or without a history of migraine. Both groups showed significant alleviation of dizziness and improvement of balance and gait. Vestibular exercises also have been shown to improve postural control during the first month after acute unilateral vestibular lesions resulting from vestibular neuronitis.
