Most have mild to moderate dementia. For the study, Black and her colleagues performed in-home assessments and surveys of 254 people with dementia living at home in Baltimore and also interviewed 246 of their informal, non-professional caregivers. They found that 99 percent of people with dementia and 97 percent of their caregivers had one or more unmet needs. Ninety percent were safety-related. Researchers report that the number of nerve cells in the vestibular system drops after the age of 55 years. This decline gets worse the older we get.

Vertigo

If you have vertigo—the feeling that you or everything around you is spinning—you probably have a problem in the labyrinth of your inner ear. The most common inner ear disorders that produce vertigo are:
  • benign paroxysmal positional vertigo (BPPV)
  • labyrinthitis or acute vestibular neuritis
  • Meniere’s disease
  • migrainous vertigo
  • acoustic neuroma

Sometimes, the calcium crystals in your inner ear break loose— either because of an ear infection, a bump on the head, ear surgery, or aging. The crystals can then travel into one of the semicircular canals. When you move your head, these misplaced crystals can shift position. If this happens, incorrect messages may be sent to your brain about your head position. BPPV comes on suddenly and powerfully.  It is the most common cause of vertigo in older people.  Moving, standing up, turning in bed, or slightly changing your head position makes it worse. The intense symptoms usually come and go, and last for less than a minute. But they tend to reappear frequently over the course of many days. Along with the vertigo, you may also experience
  • dizziness and lightheadedness
  • unsteadiness and loss of balance
  • blurred vision
  • rapid side-to-side eye movements that are not under your control
  • nausea and vomiting
  • migraine

It's called Mal de Débarquement Syndrome (MdDS) or Disembark­ment Syndrome. According to the Mal de Débarquement Syndrome  Balance Disorder Foundation, Mal de Débarquement Syndrome (MdDS) or Disembark­ment Syndrome is a rare disorder that most often develops following an ocean cruise. Less often, the perceived sensation of motion follows prolonged air or auto travel. More common in women than men, the symptoms of Mal de Débarquement Syndrome include a persistent sensation of motion (rocking/swaying/bobbing/tumbling) that is usually associated with fatigue, imbalance, and difficulty concentrating (impaired cognition). Some patients develop Mal de Débarquement Syndrome after extended boat or train travel, or other motion experience, or even spontaneously, i.e., they lack an identifiable motion event. The intensity of symptoms is highly variable among patients but may be quite disabling. Symptoms are often reduced when in passive motion such as in a moving car, airplane, or train. After remission, the recurrence rate of Mal de Débarquement Syndrome  is high and symptoms usually more severe. Unfortunately, MdDS may persist for years, is frequently misdiagnosed or undiagnosed, lacks an effective treatment, and the cause remains unknown. Mal de Débarquement Syndrome research studies at UCLA and LIBR by Yoon‐Hee Cha, MD; a neurologist, Dr. Cha have identified specific locations in the brain that are affected by Mal de Débarquement Syndrome. There is also a research study by Brian C. Clark, PhD, Director of the Ohio Musculoskeletal and Neurological Institute (OMNI) at Ohio University. Dr. Clark has defined increased intracortical excitability in MdDS patients. Hopefully, the combined results of these independent investigations will help to form the basis for a rational and effective MdDS therapy in the future. In the meantime, older individuals are encouraged to perform specific senior balance exercises to boost their stability and minimize their symptoms from Mal de Débarquement Syndrome.
