       Doctors, medical researchers and physical therapists often wonder if practicing balance exercises help people who have vertigo. Please see     http://www.exerciseforbalance.com/buy-now In discussions with patients as well as other physical therapists, I have often been a part of a conversation that asks why some people with vertigo respond quickly to treatments and why some patients recover from symptoms of vertigo over the long haul. Some healthcare professionals believe that the answer to that question lies in the site of injury or lesion that causes the vertigo. Other patients and medical professionals wonder if the location of the lesion makes a difference with treatments given to combat the feelings of dizziness and imbalance. For instance,  at the American Institute of Balance, the question was asked, "Does the location of the lesion effect Vestibular Rehabilitation treatment protocols and outcomes? Why do some patients have spontaneous central compensation and others do not? Why do some patients have a faster and more successful recovery than do others?" Although, we have recognized the location of the lesion as a critical variable, thanks to the recent publication by Becker-Bense et al (Vestibular compensation in acute unilateral medullary infarction. Neurology 80, 2013; 1103-1109) we now have physiologic documentation with PET scans. The inability of clinical diagnostic procedures, especially reduced caloric response, to differentiate labyrinthine end-organ from neuropathy or central vestibular pathway involvement has always presented a challenge. Unlike the auditory system there is no definitive clinical assessment method of identifying whether the vestibulopathy or reduced labyrinthine response is sensory or neural.  Do those patients with more peripheral or ear lesions i.e. vestibular neuritis, produce a compensation differently than those we’ve suspected a centrally mediated dysfunction i.e. at the level of the vestibular nuclei? Should we employ different Vestibular Rehabilitation Therapy protocols based on the location of the deficit? The researchers looked at two groups of patients, those with a diagnosed acute peripheral vestibular loss vs. those with radiographically confirmed acute brainstem infarct.  The resulting PET scans showed varying areas of the loci of compensation. The subjects with peripheral involvement showed compensation within the cortex and the infarct patient’s compensation was largely brainstem and cerebellar. This may help explain why numerous investigators (Balaban, Furman, Whitney, Herdman, Gans and others) have advocated the use of concentration and attention as facilitators of central compensation. Balaban’s theory of engaging the cortex throughout therapy now has been validated beyond our anecdotal experience with patients. Disorganization is difficult for any age, however hoarding and chronic disorganization in the senior population can be very challenging and very risky for health reasons. Hoarding is the accumulation and failure to discard a large number of possessions that appear to most people to be useless or of limited value. Hoarding can include items of clutter piled on the floor, which can lead to tripping and falling. Additionally,  extensive clutter can be found  in living spaces that precludes activities for which the rooms were designed.  medical researchers have found that there is significant distress or impairment in functioning caused by hoarding (Frost & Hartl 1996).  According to the Institute for Challenging Disorganization (ICD),  chronic disorganization is characterized by disorganization that has persisted for a long period of time, has a negative impact on quality daily life, has not responded to repeated self-helps, and is expected to continue into the future. Furthermore, hoarding affects 2-5% of the population in the U.S., however many people are reluctant to report the issue, especially seniors. Seniors are hesitant to seek help for a number of reasons including: lack of public awareness, cost of treatment, transportation problems, negative views of mental health treatment, and low motivation and insight. (Ayers, C.R. 2010)  The diagnosis of hoarding has been adding to the Diagnostic and Statistical Manual of Mental Disorders (DSM) in 2013. Many insurance companies will now cover the mental health therapy that is needed for hoarding clients.  Many times hoarding is triggered by a traumatic loss such as loss of a loved one, empty nest syndrome, illness, divorce, moving, or inheriting belongings from a loved one. Seniors can go through several of these life events and it may trigger the hoarding. Challenges with hoarding with seniors include a difficult time parting with belongings. Many have lived through the Great Depression and World War II and become practical and frugal. Practicality and frugality are wonderful traits, however since they do not want to be wasteful, they do not let go of things. There are many emotional and physical challenges with seniors that can occur such as dementia, physical and mental health issues such as diabetes and depression,  limited physical mobility and fall risk. If you know a loved one living in a hoarding situation, you can refer them to www.instituteforchallengingdisorganization.org (ICD).     With the recent storms crashing on the coast of California, waves have swollen to mammoth heights, requiring surfers to be in tip top physical shape and have a reservoir of balance exercises at the ready. The storm called the Pineapple Express is an atmospheric river of sorts that streams moisture from the Pacific tropics toward the West Coast. Wind gusts of up to 147 mph were reported Thursday. The devastation from the storm included a mudslide that blocked a stretch of Pacific Coast Highway with up to 18 inches of mud, Ventura County officials told The Weather Channel. The Los Angeles Times said that at least five cars were buried in mud on the Pacific Coast Highway and that the passengers of those vehicles were rescued. Remarkably, the National Weather Service confirmed that an EF-0 tornado struck a neighborhood in south Los Angeles Friday morning. The storm whipped up hurricane-force winds — and surf conditions on Lake Tahoe, with waves cresting as much as seven feet high. On the coast, waves were reported to be thirty feet high in some areas.  To surf a big wave, riders must be in excellent physical condition and have superb stability, developed by not only years of experience on the board but also time performing balance exercises on dry land.

Recently,   a video was newly released that shows extreme skier, Cody Townsend, plunging nearly 2,000 feet through a vertical chute. The ultimate skiing took place down a chute sandwiched between two ridges in Alaska’s Tordrillo mountain range. The height of the drop is taller than the Empire State Building, or five football fields stacked together. “It wasn’t actually until I dropped in and then started going and realizing how fast I was going, how narrow it was and how steep it was truly,” Townsend said. “It kind of all came together like, ‘Oh man, this is terrifying.’” The rock walls were only feet apart at the narrowest point. The two thousand foot descent took place in only sixteen seconds. Townsend said that he practiced for four years before attempting the feat. I can imagine that balance exercises were part of his preparation and fitness regime.
